Terms and Conditions of the Know Shingles campaign

You hereby grant GlaxoSmithKline Australia Pty Ltd ("GSK"), its affiliate companies and any licensees, successors or assignees, the perpetual, irrevocable, worldwide and royalty-free right to use, edit and distribute the information submitted to use, including your first name, state/territory, the age you contracted shingles and the testimonial collected below along with any other reproductions, adaptations or derivative works of the information supplied.

Furthermore, you understand that the information submitted may be retained and used by GSK, its licensees, affiliates, successors and assignees for future health and information campaigns and other purposes set out in GSK's Privacy Notice. You further understand that the information submitted on this form, may be transferred to or stored in other countries where personal information is not protected to the same extent it is protected under the laws of Australia. In granting this consent, you understand and agree that no consideration is owed to you by GSK or any other party.

In relation to any moral rights that may arise by operation of the Copyright Act 1968 (Cth) in respect of any works or other subject matter created by you in the course of sharing your story (Works), you, as an author of the Works, irrevocably and unconditionally consent, to the maximum extent permitted by law (either present or future), to GSK:

(i) using, disclosing, reproducing, copying, adapting, publishing, performing, exhibiting, communicating, renting or transmitting any of the Works or any adaptation of any of them (or any part of any of the Works or of any such adaptation) anywhere in the world:

(A) in whatever form and in whatever circumstances GSK thinks fit, including the making of any distortions, additions or alterations to any of the

Works, or mutilating or destroying any of the Works, or any adaptation of any of them (or any part of any of the Works or of any such adaptation); and

(B) without making any identification of you as an author in relation to any of them; and

(ii) doing anything or omitting to do anything in relation to any of the Works or any adaptation of any of them (or any part of any of the Works or of any such adaptation) anywhere in the world that would otherwise infringe your moral rights, or any similar non-assignable, personal rights that you might have.

(A) You acknowledge that this consent applies to all Works, whether created before, on or after the date of agreement and acknowledgement of these terms and conditions.

By providing your story and testimonial via social media or a survey function, you agree to the above use and disclosure of your personal information, and represent and warrant that the information provided is true and accurate and that you are 18 years of age or older. Your posts are subject to the terms and conditions of the relevant social media platform, including relevant terms of use and content restrictions, and you acknowledge your story must be in accordance with such terms and conditions.

GSK is required to pass details of any adverse events or product complaints to relevant regulatory authorities. If you raise an adverse event or product complaint, GSK may be required to report this, even if it has already been reported by you directly to a pharmaceutical company or the regulatory authorities. In the event of an adverse event/side effect/product complaint being raised through this initiative, you acknowledge that your personal or sensitive information may be disclosed to GSK if you have given your permission for such disclosure and that GSK may disclose your personal or sensitive information to the relevant regulatory authorities solely for the purpose of, and to the extent required to, report such adverse event.
